DentalGoodNews | March 25, 2026. The "2025 NHS and Social Care Public Satisfaction Report" released by the UK think tank The King's Fund shows that public satisfaction with NHS dentistry across the UK is only 22%, still hovering near its historical low. However, Scotland's performance is significantly better than other regions, with a satisfaction rate reaching 40%.

The satisfaction survey for healthcare services covered 1,460 respondents. The results show that 54% of Britons are dissatisfied with the current NHS dental services. Looking at regional breakdowns, satisfaction in Scotland (40%) is far higher than in England (21%) and Wales (14%). Scotland is also the only region in the UK where satisfaction exceeds dissatisfaction. But according to previous reports by DGN, 65% of the public in Scotland believe the government's investment in dentistry is insufficient, calling for more action to improve service accessibility.

This geographical disparity may stem from differences in payment models. Scotland and Northern Ireland continue to use the "item-of-service" model, where payment is based on specific treatment items provided by the dentist. In contrast, England and Wales have long relied on the "Unit of Dental Activity" (UDA) framework, where practices receive largely the same remuneration regardless of the complexity of the treatment process. This model has been criticized by some as leading to inefficient service delivery and resource misallocation.

According to previous reports by DGN, data on the pressure on UK NHS dental services shows that over 40% of dentists failed to meet their contracted UDA volumes in the past two years, collectively returning nearly £1 billion to the government. To address this crisis, the English government has confirmed that starting from April 2026, NHS dental contractors must allocate 8.2% of their contract value to urgent or non-appointment care activities. This means that for every £10,000 (approximately RMB 92,200) of contract value, 11 urgent care courses must be provided, and the payment standard for each urgent care case will be increased to £75 (approximately RMB 692).

The Welsh government has also announced major reforms. Starting April 1, 2026, Wales will formally abolish the UDA model, which has been in place for 20 years, and transition to a risk- and needs-driven time-based model. The new contract will place preventive care at its core and increase payment rates for dentists, with the standard hourly rate rising to £150. Furthermore, for NHS patients who are required to pay, their total personal contribution will be capped at £384.

The think tank notes in the report that public confidence in NHS dentistry often follows changes in system design. With Wales transitioning to a time-based system and England mandating urgent care quotas, NHS dentistry in the UK is undergoing its most significant structural changes in recent years. For practices in England still operating under the UDA framework, the successful experience of the Scottish model could become a reform reference that the government cannot ignore.
